Media Opt out Form

This form must be completed only if you do not give permission for your student to appear in news media or school and district publicity images.

Brookfield Acceptale Use Policy

The Acceptable Use Policy Form is required to be completed at the beginning of each school year by the student and their guardian. If a student does not have a current form on file, he/she will be denied access to the Brookfield Local School District's computers, network and the Internet.
